Patient Forms
 Upon entering the office our receptionist will greet and welcome you. We will request you fill out our patient forms. You may also download the forms here, complete and bring them to your first appointment. The paperwork provides us with general information about you and your condition.
Consultation
 Next you will have a consultation with Dr. Trista DeLuca to discuss your health-related problems, concerns, as well as potential treatment options. Your first visit is designed for Dr. DeLuca to learn more about you, your condition and your expectations to determine whether chiropractic care will you meet your goals.
Examination
 After your consultation, Dr. DeLuca will perform a complete chiropractic examination testing your ability to turn and bend as well as other standard orthopedic, postural, and physical examinations.
Report of Findings
 Once the initial information and examinations have been completed, Dr. DeLuca will give you a report of findings to answer the three most common questions:
- Can you help me?
- What do you recommend that I do to get better?
- How much is it going to cost?
After discussing your history, goals, examining your spine and reviewing any x-rays or tests, Dr. DeLuca will present options. If you have a condition that requires care from other providers, she will refer you. If Dr. DeLuca believes she can help you, she will recommend treatment and a wellness program.
Treatment
 Patients will generally be provided with their first treatment at the conclusion of the appointment. This may include spinal adjustments, physiotherapies, and/or soft tissue therapies. Treatment is interactive so you can express preference for the style of adjustment that you find most comfortable.
INSURANCE
Most insurance plans are accepted at our office. This includes auto accident, workers compensation, personal injury and most health insurance plans, including Medicare. Since there are so many plans and coverage varies widely, please call us (215 723-7500) to find out if your plan covers care in our office. Our staff will verify and explain your benefits before treatment.
NO INSURANCE
For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged. To learn more about our payment options for chiropractic and wellness care, call our office directly.
"ON THE JOB" INJURY (Worker’s Compensation)
If you are injured on the job, your care should be covered under your employer’s Worker’s Compensation insurance. Inform your employer of the accident and obtain the name and address of their insurance carrier.
AUTOMOBILE ACCIDENTS OR PERSONAL INJURY
If you are injured in a motor vehicle accident, your care should be covered under your auto insurance policy. Inform your carrier of your injuries. If you have had a personal injury, your care should also be covered. Please notify our office of the responsible insurance party.
